The Requirements:
1. Purchase two or more ceiling fans from The Home Depot.
2. Call 1-800-HOMEDEPOT to arrange installation and have the fans installed through
The Home Depot Home Services in participating markets between 2/7/08 and 12/31/08.
3. Mail this completed form along with a copy of the original The Home Depot sales
receipt(s) OR a completed validated copy of the Special Services Customer
Invoice(s) AND the installation agreement AND the UPC barcodes from
the qualifying installed ceiling fans to:
